Job Title: Transition Coordinator/Community Connector
Reports To: Executive Director
FLSA Status: Non-Exempt
Classification: Full-Time
Hours: Monday – Friday 9:00-5:00, occasional evenings and weekends

Position Summary:
The Transition Coordinator/Community Connector(TC/CC) is responsible for assisting individuals with disabilities who are moving from institutional settings, such as nursing homes or hospitals, to the community. The TC/CC is also responsible for supporting individuals with disabilities (who may or may not be in institutional settings) to increase their independent living skills and inclusion in all aspects of the community. The individuals served are of varying ages, backgrounds and needs.

Essential Duties and Responsibilities:

  • Assists individuals with development and implementation of an Independent Living Plan (ILP)
  • Assists individuals with finding appropriate, safe, affordable and accessible housing for those transitioning from Nursing Facilities. (May include assessing housing options, obtaining information for individual to complete housing applications and completing applications, arranging for payment of application fees, security deposits and other expenses, and overcoming housing obstacles such as credit recovery, criminal convictions)
  • Assists individuals with moving support if discharging from Nursing Facility
  • Assists individuals in obtaining sufficient medications and medical supplies if discharging from Nursing Facility
  • Ensure that medical appointments have been, or are being, made and addressing any barriers to keeping appointments
  • Assists individuals with financial tasks as necessary to ensure sustainability in the community
  • Assists individuals with coordination of benefits such as Social Security, Medicare, Medicaid, Food Assistance, Veterans Benefits as needed.
  • Assists individuals discharging from Nursing Facilities with acquiring the goods and services needed to improve and maintain living condition in the community as necessary
  • Assists individuals with linkage to service providers and community resources
  • Assists individuals with linkage to potential employment options as needed
