Dear Potential Research Participant:

We are inviting you to participate in a study "Investigating the Perceptions of Existing and Newly Created Accessibility Symbols" about current issues related to the perceptions about the familiarity and usability of existing and newly created symbols as they relate to accessibility and ease of use for people with and without disabilities in a variety of settings.

To participate in this study, you must be 19 years of age or older and participation in this study involves completing the survey in this link, Accessibility Symbols Survey. Completion of the survey should take approximately 15 minutes. Your participation in this study is voluntary and you may withdraw your consent at any time by exiting the survey.

Your responses will be confidential. The list that links codes with programs will be stored separate from the data. Data will be reported in aggregate form so your responses will not be identifiable in any publications resulting from this research.  Information identifying programs will not be reported in the results. By completing and submitting the survey you are giving consent for your responses to be included in the study.

The risks of this study are minimal and there are no direct benefits to your participation.

A summary of the study results will be available upon request. If you have any questions regarding the nature of the study or research procedures, please contact Elizabeth A.
